Otoplasty corrects prominent ears (also known as “protruding” or “bat ears”) by repositioning them at a normal distance from the skull. It is a quick surgery, with no visible scar (the incision is hidden behind the ear) and a permanent result: one of the few cosmetic surgeries that can be performed from age 6.

What is otoplasty?
It's cosmetic surgery of the outer ear. It mainly treats:
- Prominent ears («bat ears», «sticking-out ears»)
- Absent antihelix (the ear's natural inner fold)
- Enlarged conchal bowl
- Asymmetry between the two ears
- Torn earlobes (from heavy earrings)
- Microtia / small ears (reconstructive techniques)
What is the best age?
The ear reaches 85% of its final size by age 6. That is why it is the cosmetic surgery that can be performed earliest:
- Ages 6-10: the ideal age for children. Prevents bullying at school. Brief general anesthesia.
- Teenagers: very common at ages 14-17, after the pubertal growth spurt.
- Adults: no age limit. Local anesthesia with sedation, 1 hour of surgery.
Surgical technique
Dr. Martínez uses an otoplasty technique with permanent sutures and cartilage reshaping. Steps:
- Incision hidden in the fold behind the ear (never visible from the front or side)
- Cartilage access: it is weakened in a controlled way so it folds like a normal ear
- Permanent sutures that keep the antihelix projected
- Minimal posterior skin resection
- Compression bandage 5-7 days
Scar and natural result
The scar is completely hidden behind the ear, in the natural fold. At 3-6 months it is practically invisible. The result looks natural because the original anatomical curvature is respected — the goal is not to “pin the ears to the skull,” but to restore their correct position and angle.
Recovery step by step
- Day 1-7: continuous compression bandage (turban), mild pain controlled with oral pain medication
- Day 7-10: switch to an elastic night bandage for 4 more weeks
- Week 2: absorbable stitches removed, back to school/office
- Weeks 3-4: sleep on your back (not on your side) to avoid folding the ear
- Months 2-3: contact sports allowed with a soft guard
- Months 6-12: stable final result, invisible scar

Frequently asked questions
How much does otoplasty cost in Lima?
At Dr. Martínez's practice it costs from S/ 8,000 (≈ $2,200 USD) for adults under local anesthesia. For children under general anesthesia it costs from S/ 8,500. It includes surgeon's fees, anesthesiologist (if needed), operating room, tests and follow-up.
Is it a painful surgery?
No. Post-operative pain is mild and well controlled with acetaminophen for the first 2-3 days. Children tolerate it very well with the compression bandage.
Can my 7-year-old have the surgery?
Yes, from age 6-7 the ear has already reached 85% of its final size. Operating before starting school helps avoid bullying and self-esteem issues. The surgery is the same, with brief and safe general anesthesia.
Are there visible scars?
No. The scar is completely hidden in the crease behind the ear and cannot be seen from any front or side angle. At 6 months it is practically imperceptible.
Is the result permanent?
Yes, it is permanent. Permanent sutures hold the new position for life. Only in aggressive contact sports can there be minimal «relapse» (<5% of cases).
When can he return to sports?
Non-contact sports (cardio, cycling, swimming) at 3-4 weeks. Contact sports (soccer, basketball, martial arts) from 2-3 months, ideally with a protective sports headband.
Is one ear or both operated on?
Usually both, even if only one is visibly prominent, to maintain symmetry. This is evaluated case by case at your consultation.
