What is a mastopexy?
It's breast-lifting surgery. It doesn't change breast size (that's what augmentation or reduction does); instead it repositions the tissue higher and removes the excess skin left after pregnancy, breastfeeding, weight loss or natural aging.
It is performed under general anesthesia at an accredited clinic, as an outpatient or with a one-night stay depending on the case.
Types of breast lift by degree of ptosis
- Periareolar (donut): mild ptosis, scar only around the areola
- Vertical (lollipop): moderate ptosis, periareolar scar + vertical scar down to the fold
- Anchor (inverted T): severe ptosis; periareolar + vertical + horizontal scar in the crease
- Mastopexy with augmentation: any of the patterns above + implants or your own fat to restore volume
When do you need a mastopexy?
There's a simple test: if the nipple sits below the inframammary fold when you're standing, there is ptosis. Other indicators:
- Nipples pointing downward
- Stretched / enlarged areola
- Breast skin with stretch marks and loss of elasticity
- Breasts that «deflate» when you take off your bra
Breast lift with or without implants
The decision depends on the breast's residual volume:
- No implants: when there is enough glandular tissue, it is simply repositioned
- With implants: when the upper pole is deflated (inverted teardrop shape)
- With autologous fat: a natural alternative; requires simultaneous liposculpture to harvest the fat

How much does a mastopexy cost in Lima?
At Dr. Martínez's practice it costs from S/ 14,000 (≈ $3,800 USD) without implants. With implants, the cost of the implant is added (S/ 2,800 – S/ 4,500 depending on brand and profile). It includes surgeon's fees, anesthesiologist, operating room and follow-up.
Does a mastopexy change breast size?
Not directly. Mastopexy alone repositions; it does not change volume. If you want more volume, it is combined with implants or fat transfer. If you want less volume, it is combined with breast reduction.
Are there visible scars?
There are scars, and their pattern depends on the degree of ptosis: periareolar (only around the nipple), vertical (down to the breast fold) or anchor (inverted T). At 12-18 months they fade and follow the breast's natural folds.
Will I be able to breastfeed afterwards?
Most modern techniques preserve the milk ducts and sensation. Breastfeeding is possible, although we talk honestly during the consultation about the individual percentages for the technique recommended for you.
How long does a mastopexy result last?
Between 10 and 15 years in patients with good skin quality, no later pregnancies and a stable weight. Gravity keeps working, but the descent starts from a higher position.
When can I exercise?
Walking from day 2, light cardio at 3 weeks, lower-body weights at 4 weeks, upper-body weights at 6-8 weeks. Any impact before then can compromise healing.
Can it be done after a pregnancy?
Yes, ideally wait 6 months after you finish breastfeeding so the breast returns to its final shape. We do not recommend surgery if you plan another pregnancy in the short term.
Will I have nipple sensation?
In 85-90% of patients, sensation is preserved or returns within 6 to 18 months. In a smaller percentage it may remain permanently reduced, which is discussed before surgery.

Post-op anti-embolism stockings — S/ 120. Graduated compression, stronger at the ankle and lighter going up, to support venous return and reduce the risk of thrombosis during and after surgery. One standard size.
