What is a BBL and why is it done with ultrasound?
The BBL (Brazilian Butt Lift) is gluteal fat transfer: fat is suctioned from areas where there's excess (abdomen, flanks, back) and injected into the buttocks. The MODERN, SAFE technique requires:
- Intraoperative ultrasound to visualize the subcutaneous plane and avoid the muscle
- Blunt-tip cannula (not sharp)
- Injection in motion (never static)
- Subcutaneous plane only, never submuscular
This reduces the risk of fat embolism (the leading cause of death from BBL) to practically zero.
Types of buttock augmentation
- BBL with your own fat: the most natural and most in-demand option. Requires enough donor fat (ideal BMI 22-28).
- Silicone buttock implants: for very slim patients without enough fat. 5-6 cm intergluteal scar.
- Implants + fat combination: implants for structural volume + fat to refine the transition. A more natural result in very thin patients.
- Biostimulators: a non-surgical alternative with modest results; lasts 18 months (Sculptra/Radiesse).
Who is a candidate?
Ideal candidates for BBL with your own fat:
- BMI between 22 and 28 (with enough donor fat)
- Age 25-50 (better skin and fat quality)
- No clotting disorders
- Realistic expectations (a viral «before/after» isn't achieved in a single surgery)
- Commitment to recovery: 14 days without sitting directly on the buttocks
How much volume can be transferred
A safe session transfers between 300 and 800 cc per side. More volume requires a second session. About 60-70% of the transferred fat survives at 6 months (depending on technique + post-op care). The rest is reabsorbed in the first 8 weeks.
Recovery step by step
- Day 0-14: do NOT sit directly on your buttocks (use a special BBL cushion); sleep face down or on your side
- Day 1-3: compression garment 24/7, manual lymphatic drainage from day 2
- Weeks 2-4: back to the office with a cushion, daily walks
- Months 1-3: gradual return to exercise; avoid heavy squats
- Month 6: stable final result; the fat that survived is now consolidated

Is a BBL safe?

How much transferred fat survives?
Between 60 and 70% at 6 months. The reabsorbed fat (30-40%) is lost in the first 8 weeks. The final, stable result is seen at 4-6 months.
When will I see the final result?
First glimpse at 3-4 weeks (once swelling goes down). True result at 3 months. Stable final result at 6 months.
Should I gain weight before a BBL?
Only if your BMI is below 22 and there is not enough donor fat. Dr. Martínez will tell you in consultation whether you need a period of controlled weight gain before surgery.
Can it be combined with other procedures?

How long does the result last?
It's permanent for the fat that survived (fat cells don't regenerate). But volume can change if you gain or lose a lot of weight. Keeping a stable weight (±5 kg) preserves the result for decades.

During surgery and the hours of rest afterward, circulation in the legs slows down and the risk of blood clots increases. Anti-embolism stockings apply graduated compression, stronger at the ankle and lighter further up, which helps keep blood flowing and reduces that risk.
Post-op anti-embolism stockings — S/ 120. One standard size, with a non-slip silicone band that keeps them in place without marking the skin. It's best to buy them before surgery, not the day after.
