Size really matters!
Dr. Martínez talks at length with patients during the consultation. Most patients of average height with a slim to medium chest choose sizes between 300 and 400 cc. However, some patients may opt for smaller or even larger sizes. Large sizes above 500 cc are scarce in our country, as are saline implants. Generally, if you want these implants, they need to be reserved in advance. At one point, Dr. Martínez used 3D computer programs to simulate the breast implant during the consultation, but this was discontinued because the images obtained did not correlate well with the patient's body or with the actual final result. The approach Dr. Martínez uses is periareolar. The placement plane can be retroglandular, subfascial or retromuscular, depending on the patient's anatomy. The implant surface can be smooth, textured, microtextured or polyurethane. The choice of implant type is also made together with the patient. The different textures exist because of the capsular contracture that some women may develop over the years after their surgery.
Dr. Martínez's personal philosophy on capsular contracture
The exact cause of capsular contracture is unknown. Some believe texture significantly reduces the contracture rate, while others believe it is caused by bacterial contamination of the pocket. After reviewing scientific studies and observations in our own patients, Dr. Martínez is confident that polyurethane implants do significantly reduce the contracture rate. At the same time, using antibiotic irrigation solution in the implant pocket has a similar effect. When the implant size the patient wants does not exceed 400 cc, a polyurethane implant can be used. When the desired volume is larger, a smooth or microtextured implant is preferred. Capsular contracture is treated by replacing the implant, removing the capsular tissue found, enlarging the pocket, using internal antibiotics and grafting fat around the implant. If the implant is in the subglandular plane, it can be moved to the retromuscular plane.
Breast implants

Placing a breast implant may not seem like a difficult procedure. However, achieving a natural-looking breast with an implant is a challenge that few surgeons can meet, Dr. Martínez among them. The surgery is performed under general anesthesia, and an incision is made to insert the implant beneath the breast gland or the pectoral muscle. Where is the implant placed?
There are 3 planes: subglandular (over the muscle), submuscular (under the pectoral muscle), and dual-plane (mixed). Dr. Martínez makes her recommendation at your consultation based on how much breast tissue you have, your skin quality and the result you want.
Where is the scar?
There are 3 approaches: periareolar (around the areola, the most hidden), inframammary (in the fold under the breast, the most common) or transaxillary (in the armpit, leaving no mark on the breast). The choice is made at your consultation based on your anatomy and preference.
How long do implants last?
Modern implants have no expiration date. They can last 15-20 years or more without needing replacement, as long as there are no complications (capsular contracture, rupture, a change in aesthetic preference).
Will I be able to breastfeed?
Yes, in most cases, especially with the inframammary or axillary approach. The periareolar approach carries a slightly higher risk of affecting breastfeeding. In consultation we honestly assess your case if you are planning future pregnancies.
When can I return to exercise?
Walking from day 2. Light cardio at 3 weeks. Lower-body weights at 4 weeks. Upper-body weights only at 6-8 weeks. Any impact before then can compromise healing.
Is nipple sensation preserved?
In 90% of cases it is preserved or returns within 3 to 12 months. In a smaller percentage (5-10%) it may remain permanently reduced, which is discussed before surgery.
